🔹 Please complete and return this form by June 1st, along with a min. deposit of $40 to secure your spot.
🔹 One (1) parking pass will be provided per reservation.
🔹 All vendors must submit a completed ST-19 form.

📌 Important: Ensure your Certificate of Compliance is fully completed, signed, and includes your Minnesota Identification Number. 

📝 Note: Machinery setup is allowed starting the Saturday before the fair and must be completed by Tuesday at noon.
